At Memory Balance therapy, we specialize in private, client-centered cognitive assessments designed to support brain health, memory, and daily living skills. Our goal is to help adults and older adults maintain independence, safety, and quality of life.

All services are provided by a registered Occupational Therapist and are eligible for reimbursement under many extended health benefits plans.

🧠 Cognitive Screenings

$250 (1.5-hour session, includes written summary)

A brief, standardized assessment to check key cognitive areas like memory, attention, executive functioning, and language.

  • Pre-screening phone call (15 minutes)
  • Standardized cognitive test (e.g., MoCA)
  • Immediate verbal feedback
  • Written summary with recommendations

🛠️ Functional Cognitive Assessments

$375 (2-hour session in client’s home + written report)

Assessing how cognitive challenges affect daily activities like medications, meals, and finances.

  • Interview to understand routines
  • Practical tasks (e.g., snack prep)
  • Risk identification
  • Detailed report and recommendations

🏡 Home and Community Safety Assessments

$400 (2-hour home visit + report)

Home safety and cognition review to support aging in place.

  • Home visit and assessment
  • MoCA screening
  • Environmental safety evaluation
  • Written improvement suggestions

📚 Memory and Aging Support Packages

$550 (Assessment + 3 sessions)

Strategy sessions for those with mild memory concerns.

  • Cognitive assessment
  • 3 follow-up strategy sessions
  • Personalized workbook
  • Family support guidance

🔄 Cognitive Wellness Check-Ups

$200 (1-hour)

A proactive “brain health” check for adults or those with family history.

  • Brief screening
  • Review of concerns or changes
  • Short written summary

👨‍👩‍👧 Family and Caregiver Education

$150 (1-hour coaching session)

Support for families around communication and understanding cognitive changes.

  • Understanding cognitive decline
  • Memory and environment tips
  • Communication strategies
  • Can be bundled with other services

Important Notes:

  • All services are billed privately.

  • Extended health benefits often cover occupational therapy services — invoices will include OT registration number.

  • Travel fees may apply for home visits beyond Burlington/Oakville area.
